GP working across PCN locality in Home Visiting Service .Derbyshire Dales PCN is looking to recruit an experienced GP who is enthusiastic & forward-thinking to join our Home VisitingService on a permanent contract .You will be required to work an agreed number of sessions across the areas that the DDPCN HVS supports.DDPCN works in partnership andprovides the home visiting service ( to Friday) for the seven GPPractices within our locality namely:Evelyn Medical Centre, HopeEyam SurgeryBaslow Health CentrePeak and Dales Medical Partnership (Bakewell &Tideswell)Darley Dale Medical CentreImperial Road Group SurgeryLime Grove Medical CentreHannage Brook Medical CentreAshbourne Medical Practice.Henmore Health ( Ashbourne & Brailsford).We are a very supportive PCN &will support you to work across the different surgeries and will have a lead GP/Clinical Director.This is a great opportunity to get toknow our practices working across the north ,central and south Derbyshire Dales areasand will provide the successful candidate with varied experience.Main duties of the jobTo perform as an autonomous general practitioner workingindependently and with other health care professionals to assess, diagnose andtreat the conditions of patients that have been triaged at practice level andrequire an acute /same day home visit or a proactively assessed visit thatfalls outside of the urgent criteria.To provide expert professional advice to patients, carersand colleagues and ensure the maintenance of clinical excellence. To beinvolved in the provision of education and training of other staff.To be professionally accountable and responsible for allelements of clinical practice in accordance with the GMC code of conductensuring the safety of patients and quality of their care is primary.The post holder will need to be capable of independenttravel across the Derbyshire Dales area and work flexibly to meet the needs ofthe Business.About usDerbyshire Dales PCN is made up of 11 GP Practices as listed above .There is an excellent road infrastructure and access to and from the M1, A6 and A38 is easily achievable .You will be working in the beautiful Derbyshire Dales which includes the well known tourist attractions that are scattered around the locality . It is a fantastic place to work with many benefits that are naturally derived by working in the Dales .The ethos of DDPCN is to support all our staff in order to maximise every opportunity to deliver a high quality of patient care on behalf of our stakeholder practices .The management team at DDPCN have a 'can do' attitude and as an employee you will be supported by an excellent clinical and back office team each day .Job responsibilitiesClinical responsibilitiesIn accordance with the Home Visiting Service (HVS ) rota, as agreed, the post-holderwill make him/her-self available to undertake a variety of duties includingconsultations, telephone consultations and queries, visiting patients at home,checking and signing/authorising repeat prescriptions and dealing with queries,paperwork and correspondence in a timely fashion.Making professional, autonomous decisions in relation topresenting problems, whether self-referred or referred from other health careworkers within the organisationAssessing the health care needs of patients withundifferentiated and undiagnosed problemsScreening patients for disease risk factors and early signsof illnessIn consultation with patients and in line with currentpractice disease management protocols, developing care plans for healthProviding counselling and health educationAdmitting or discharging patients to and from the caseloadand referring to other care providers as appropriateRecording clear and contemporaneous consultation notes toagreed standardsCompiling and issuing computer-generated acute and repeatprescriptionsPrescribing in accordance with the practice prescribingformulary (or generically) whenever this is clinically appropriateThe post-holder will be required to visit patients in anemergency if clinically indicated.The post-holder will undertake fair share of home visitingsessions per week as allocated.You will be experienced and be competent using the clinical platforms of EMIS and Systm1.Person SpecificationQualificationsExperience of working in a primary care environmentExperience of continued professional developmentExperience of QOF and clinical auditNot previously been substantively employed at a GP PracticeGeneral understanding of the primary care contractFull GMC registrationNational Performers List registration (England), noting that confirmation on this list is adequate for DBS confirmationAppropriate indemnity (MPS/MDU)Evidence of current validationEvidence of last appraisalEligibility to practice in the UK independentlyQualificationsQualified GPMRCGPGeneral Practitioner (Certificate of Completion of Training CCT)SkillsExcellent communication skills (written and oral)Clear, polite telephone mannerCompetent in the use of Office and OutlookEffective time management (planning and organising)Ability to work as a team member and autonomouslyExcellent interpersonal skillsProblem solving and analytical skillsAbility to follow clinical policy and procedureExperience with audit and able to lead audit programmesExperience with clinical risk managementFull UK Drivers license.Experience of medicines managementExperience of ICB initiativesDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
