County Durham & Darlington NHS Foundation Trust Acute Intervention Team Nurse Practitioner Closing date: 22 December 2025 We are looking for an experienced and proactive Nurse Practitioner to support acute and deteriorating patients across our Trust sites. The successful candidate will work as part of a multidisciplinary team to deliver high‑quality care and provide education and leadership for ward staff. Responsibilities Prioritise and manage patients with clinical deterioration, ensuring timely assessment and intervention. Support ward teams in delivering the right care at the right time in the right place. Use physiological track and trigger tools within the Electronic Patient Record to identify deteriorating patients. Assist with audit projects, update guidelines and policies within the Acute Intervention role. Provide support for ward staff caring for dying patients, complex DNACPR decisions and effective communication with patients and families. Assist the anaesthetic team in stabilising critically ill patients across the hospital. Act as a member of emergency response teams, taking a lead role in the Medical and Paediatric Emergency Teams when required. Lead the Cardiac Arrest Team and other rapid response initiatives. Use multi‑professional collaboration to enhance patient safety and care quality. Provide educational input through bedside teaching, pre‑ceptorship and mentoring of junior staff. Maintain accurate, contemporaneous records of patient condition, treatment and progress. Participate in audits, evaluate findings and implement changes to improve patient outcomes. Qualifications Degree or significant progress towards a relevant subject. Significant experience in acute or critical care settings. Comprehensive knowledge of acutely unwell and deteriorating patients. Evidence of ongoing professional development. Physical assessment skills. Nurse Prescriber, Advanced Life Support, and ALS/EPLS provider or instructor. Special Skills & Knowledge ILS provider and ALS/EPLS instructor or candidate. Experience teaching junior colleagues in a clinical environment. Teaching qualification (998 or C&G) or relevant experience. Ability to plan and prioritise workload and make complex decisions. Demonstrated critical thinking and experience in audit processes. Statutory Registration Registered Nurse. Special Requirements Ability to travel across Trust sites to meet the responsibilities of the post. Experience Background in acute care (e.g., ED, AMU, ITU). Excellent communication skills with patients, families and the multidisciplinary team. Experience in palliative care, supervision and teaching of junior staff. Capability to manage complex appraisal and audit projects. Disclosure and Barring Service check will be conducted as per the Rehabilitation of Offenders Act (Exceptions Order) 1975.
