The post holder is acting withintheir professional boundaries, will provide care for the presenting patientfrom initial history taking, clinical assessment, diagnosis, treatment andevaluation of care. They willdemonstrate safe, clinical decision-making and expert care, includingassessment and diagnostic skills. Thepost holder will demonstrate critical thinking in the clinical decision-makingprocess. They will work collaborativelywith the general practice team to meet the needs of patients whether viatelephone, face to face or in the patients home or Care Home; supporting thedelivery of policy and procedures. In order to work at this level, NMCrequirements for advanced practice must be met, with experience as anIndependent nurse prescriber.Main duties of the jobAssess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods.Assist GPs with the care given in local Nursing Home(s) including weekly ward rounds.Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.Work with patients in order to support compliance with and adherence to prescribed treatments.
