This role supports patients with cancer who are acutely unwell. An experienced practitioner with decision-making autonomy, responsibility and accountability within agreed guidelines and protocols, who contributes to the leadership, development and supervision of others. The practitioner works collaboratively with others or independently to meet the needs of the patients and their families. Main duties of the job Main duties of the job The post holder will undertake triage of the patient after referral from medical colleagues across the Trust. You will advise and support clinical colleagues on the investigation, care, and management of this group of patients affording access to faster and better care of patients with complications of cancer (early recognition, better treatment, rapid referral to other specialist teams as appropriate including oncology and palliative care). The acute oncology service supports newly diagnosed cancer patients and patients with a cancer of unknown primary and also are involved MUO patients. You will work cross site and work independently as well as part of a team. The post holder will be encouraged to collate and present service data at local and national meetings. They will be responsible along with the team for delivering and leading a comprehensive AOS within the Trust in line with the National Cancer Peer Review Programme (2011). The Core competencies for AO teams are now defined and adopted nationally (links on Macmillan site) and all AO CNS's should be a level 3 AO practitioner and should evidence continuing professional development in AO to maintain their level 3 competency. Detailed job description and main responsibilities Part of a team working in an area of practice, providing expert professional advice to others across the Trust within agreed guidelines and protocols, including medical colleagues and other professional groups when required. Perform comprehensive specialist assessment of patients, plan, treat and evaluate care within agreed guidelines and protocols according to evidence-based practice and changing health needs. Communicate complex specialist information to medical colleagues and others. Refer patients to appropriate clinicians, in the Trust or externally, within agreed guidelines and protocols, according to assessment of patient need and evidence-based decision. Prioritise patient care, supervising and allocating one’s own workload, monitoring and evaluating effectiveness of care, including discharge of patients, or referring on, within agreed guidelines and protocols. Perform specialist clinical skills in the treatment and overall management of patient care, including requesting and interpreting investigations, within agreed guidelines and protocols. Provide a comprehensive specialist service, accepting accountability and responsibility for the management and effectiveness of the service on a day-to-day basis. Collect, collate, evaluate and report on information, including communicating detailed information to clinical colleagues and others, maintaining accurate patient records and documentation. Promote patient and public involvement in the improvement of services and involve patients and carers/relative in the planning and delivery of their care. Work collaboratively with other professionals and agencies to ensure patient needs are met, especially in relation to patient flow, care pathways and discharge arrangements. Analyse and act on information to proactively manage and maintain NHS targets and standards. Person specification Communication Advanced Communication Experienced Nurse with acute experience in a cancer field Broad range of clinical experience in different settings, post registration experience at a senior level Good Communicator - keen to do Advanced Communication study Ability to Lead and influence change Skills Relevant specialist clinical skills with post registration experience, at Band 6 or above in cancer, palliative care, community nursing or other relevant speciality non-medical prescribing or prepared to work towards Experience Recent evidence of study in speciality or related field i.e. cancer, palliative care or community nursing Knowledge of equality, diversity, and rights Key Messages Not all NHS roles are eligible for sponsorship.
