Practice Plus Group is seeking dedicated healthcare professionals to join our Out of Hours team and make a real difference in the community.Practice Plus Group is a leading provider of urgent care services across the UK, committed to delivering safe, effective, and compassionate care to patients when they need it most — outside of regular GP hours.In OOH we assess and treat patients who require urgent unscheduled intervention suitable for treatment by general practice clinicians when the GP surgeries are closed. Our referrals come from either 111, via a dedicated health care professional line used by paramedics and district nursing teams or from the AED or MIU.What you'll be doingDeliver comprehensive clinical assessment, diagnosis, and treatment for patients presenting with urgent healthcare needs.Conduct consultations from the clinical base, providing face-to-face care in a fully equipped environment.Undertake home visits to patients requiring assessment or treatment in their own residence, offering a personalized and compassionate approach.Manage telephone triage, providing expert clinical advice, risk assessment, and directing patients to appropriate care pathways.Work autonomously while collaborating closely with a multidisciplinary team to ensure seamless, patient-centred care.Maintain accurate clinical records and adhere to Practice Plus Group policies and NHS guidelines to uphold high standards of care.What we’ll look for in youRegistered Nurse with MSc in Advanced Clinical Practice or equivalentExperience within Out of Hours, Primary Care GP Surgery, or Urgent Treatment CentreHolder of V300 / Non-Medical Prescribing qualification and minimum 6 months experience post qualification.Ability to autonomously see and treat patients of all agesConfident clinical decision-maker with strong assessment skills.Excellent communicator with a patient-centred approach.Flexible, self-motivated, and committed to ongoing professional development.If you would like to talk to our recruiter for further information, or for a full, comprehensive Job Description, please contact George Robinson - George.Robinson@practiceplusgroup.com.Please note that offers of employment are subject to receipt of satisfactory references and a DBS check.Practice Plus Group is committed to creating a diverse environment and we are proud to be an equal opportunity employer. #J-18808-Ljbffr
