Your role goes beyond managing referrals and assessing needs you ll empower clients to maintain their accommodation and take positive steps toward lasting change. Working across our supported housing services in beautiful Bath, you ll support a diverse and complex client group, tackling challenges with resilience, creativity, and empathy. Collaboration is key you ll work closely with partners such as the National Probation Service, Police, and local support agencies, ensuring a joined-up approach that truly transforms lives. Responsibilities include: Manage referrals and outcomes: Track and progress referrals into the service, ensuring smooth transitions and positive outcomes for service users. Assess needs and risks: Complete thorough needs and risk assessments, leading to tailored support plans that meet individual requirements and help reduce re-offending. Onboard new clients: Ensure clients understand their rights, responsibilities, and the conditions of their licence agreement when joining the service. Deliver high-quality support: Provide a person-centred service that is strength-based and trauma-informed, promoting empowerment and resilience. Understanding of the causes of offending, homelessness and social exclusion, and the approaches which seek to address these.
