Responsibilities : Deliver assessments, care planning, and risk management under Self Directed Support and Adult Support and Protection legislation. Provide tailored information, advice, and support to service users and carers. Develop and coordinate outcome-focused care packages with internal and external providers. Maintain accurate records and participate in supervision, training, and team duties. Promote independence, choice, and empowerment while identifying and reporting unmet needs.
