Social Worker – Rochdale – Adult Community team

Job Title: Social Worker – Neighbourhood Team Location: Rochdale Rate: £35.00An exciting job role has recently become available at Rochdale Metropolitan Borough Council. They are looking for a new member for their Adult Community Team. What will your responsibilities be? In this role, you will provide person-centred assessments of need at a level of complexity appropriate to the role, such as addressing multiple social or health needs and managing complex care and support arrangements as allocated by your line manager. You will work with people in crisis, where urgent support arrangements are needed, and make decisions regarding mental capacity and/or the Mental Health Act. Additionally, you will support individuals to receive care and treatment in the community rather than inpatient settings, arranging effective discharge pathways and facilitating community connections, housing, and discharge arrangements. Using active listening skills, observation, and communication, you will develop supportive and therapeutic relationships with adults, their families, and informal networks, exploring their situations and collaboratively finding ways to move forward.
