The candidate will be working with health professionals such as district nurses, therapists etcCandidates should have experience and knowledge of:Care act assessment,Strength based support planning BIA and capacity reviews,SafeguardingCandidates must have 2 years’ experience within a specific neighbourhood team - otherwise 3 years’ experience+ is requiredCandidates must be local to Bolton and able to commit to working in Bolton as role is hybrid but must be able to make it into the office or complete a visit at short notice.
