What you will do Conduct Care Act assessments and create bespoke care support plans for residents of Bolton, ensuring they live well at home. Engage in strength-based support planning, helping adults and their families understand and navigate their care options. Perform Mental Capacity Assessments and participate in Best Interests meetings. Manage cases involving complex social needs and family dynamics, offering information, advice, and transferring cases to other teams when necessary. Execute safeguarding responsibilities by assessing risks and leading Safeguarding Adult Enquiries as required. Collaborate with Social Workers, Occupational Therapists, Nurses, and Mental Health Practitioners to provide high‑quality care services. Attend case conferences and represent the Council at court, completing all related administrative tasks efficiently. Why Bolton?
