client/child: Ivy is 13 years old and lives at home with her Mum and Dad. She is an engaging and active young lady who enjoys riding her bike, sensory play, and spending time with her friends. Ivy has cerebral palsy and epilepsy; she is dependent on 2:1 support to meet all her needs. Ivy is exploring different communication aids to support her non-verbal communication; she is a wheelchair user and follows the ethos of Conductive Education. Her home is fully adapted to meet all her needs, including ceiling track hoists, equipment (walker etc.). Ivy has a team of carers providing day to day support, and now require a SEN / SEND Nanny to join to take on a more dynamic position, supporting Ivy, her team, and parents. Overview of role: The SEN / SEND Nanny will be the link between Ivy’s parents, school, care team, MDT, and other professionals to ensure Ivy is striving, thriving, and receiving the highest standard of care and support. The role will involve providing practical support to Ivy, supporting her alongside a team member with her ADL’s, therapy input, social aspect, medical needs, supporting in the school setting etc. It will also include some household tasks such as light chores, running errands, shopping, and enabling Ivy’s Mum to get back to work herself. You will manage Ivy’s diary, appointments, and liaise with her care and case management team. This role is all-encompassing and evolving; Ivy requires an advocate and one person who can pull together all sides to ensure everyone is on the same page when it comes to her care. You will wear many different hats each day, therefore you must be able to step up and take on each task at hand.
