Job Title: Adult Social Worker – Assessment and Intake TeamLocation: SouthwarkRate: £32 per hourWe have an exciting opportunity for a dedicated Adult Social Worker to join the Assessment and Intake Team for our client based in Southwark. What will your responsibilities be?You will be part of a vibrant and fast-paced team based in the community, providing an initial assessment function for the Older Persons and Physical Disability Service. Your role will involve completing Care Act Assessments, putting in place long-term packages of care, undertaking carers assessments, conducting urgent assessments for care and respite, and assessing for placement in residential and nursing home settings.
