About the role Seeking a Social Worker to work within the Reablement Service. This role offers a unique opportunity to thrive in a multi-disciplinary environment, collaborating closely with health professionals. What will your responsibilities be? Goal setting Completing Care Act assessments and devising care and support plans Completing reviews Completing Mental Capacity Assessments Chairing Best Interests meetings Benefits: Supportive manager and team Hybrid working Qualifications and Experience: To be successful in this role, you must have Social Work England registration, be eligible to work in the UK, hold a Social Work degree, and have post-qualified experience.
