One of our leading NHS clients is looking for experienced mental health nurses to cover the shifts in the community hospitals in Wales. We are seeking applications from Registered Mental Health Nurses who have experience of working in community services. Description: The post holder will be responsible for the assessment of care needs, the development of programmes of care and the implementation and evaluation of these programmes of care for a defined clinical caseload including the role of a Care Coordinator within the Care Treatment Planning under the Mental Health (Wales) Measure . The post holder is required to work without direct supervision for their delegated caseload. The post holder will have effective communication skills and develop positive working relationships between all healthcare professionals, statutory and voluntary agencies. Maintaining good communication across all boundaries. Main duties will include : To adopt a systematic approach to all patient care, taking responsibility for all stages of assessment, planning , implementation and evaluation of patient care., working explicitly within the requirements of the Welsh Mental Health Measure. Management of a delegated caseload. Participate as an active member of the CMHT and liaise with other agencies and teams as required To recognize the involvement of carers and to provide and identify support for them as appropriate To participate in the provision of Duty Officer. Requirements: Experience in mental health Qualifications Experience in community mental health Wide range of experience within a service related setting Demonstrates achievements in developments of care standards/procedures guidelines Demonstrates effective team working Use of I.T. skills Demonstrates research and audit experience Clinical Audit experience Welsh Speaker (Level 1) is an added advantage.
