Job Title: Social Worker – Discharge to Assess (D2A) TeamLocation: LancashireRate: £33-£35 per hour. Liquid Personnel is currently recruiting for a Social Worker to join a busy Discharge to Assess (D2A) Team based in Lancashire. What will your responsibilities be? You will gain experience within adult social care and complete mental capacity assessments. You will set up packages of care and support planning. At all times, you will identify areas of need and/or risk of harm and ensure appropriate safeguarding practices and measures are observed and in place.
