Job Title: Social Worker – Adult Community teamLocation: Calderdale CouncilRate: £35 per hourLiquid Personnel is currently recruiting for a Social Worker to join a busy Adult Community Team based in Calderdale. What will your responsibilities be?In this role, you will assess and review needs under the eligibility criteria of adult social services. You will coordinate the allocation of safeguarding referrals and monitor their progress. You will design individual care packages with the service user, family, and/or carers within a community setting. You will request funding for packages of care via the resource allocation group through weekly presentations and negotiations. You will undertake safeguarding investigations within the community and residential settings. You will chair professional and formal meetings in relation to service users, such as multi-disciplinary meetings and case conferences. You will contribute to team duty rotas to provide prompt and emergency support as required. You will maintain professional knowledge and skills through continuous professional development, reflective practice, training, and relevant post-qualifying awards under the PQ framework. You will maintain knowledge of relevant research, government guidance, legislation, and best practices in your specialist areas of practice. Additionally, you will contribute to the continuous improvement and development of adult services, including providing oversight and support to other professionals.
