Job Title: Social Worker – Hospital Discharge teamLocation: Calderdale CouncilRate: £40 per hourLiquid Personnel is currently recruiting registered and experienced Qualified Social Worker Team Leader to join a busy team at a Calderdale based Hospital Discharge Team. What will your responsibilities be?In this role, you will ensure effective screening of referrals against eligibility criteria and creative resource deployment. You will prioritize and assign work based on urgency and complexity, utilizing team resources effectively. You will manage and deploy resources within budgets to meet policy objectives and service priorities, communicating effectively with the team. You will review and evaluate Support Plans in accordance with guidelines, making adjustments for changing needs. You will implement a systematic approach to health needs assessment, planning, implementation, and review. Additionally, you will advise Team and Service Managers on expenditure for community care services exceeding guidelines.
