Job summary An opportunity has arisen at Greens Norton & Weedon Medical Practice for an enthusiastic, committed & caring Advanced Nurse Practitioner (ANP) to join our well-established two site practice for 30 hours per week (over 3-4 days).Working within the NMC Code of Conduct and utilising primary care skills and experience, working as part of our duty team, the post holder will provide advanced nurse interventions and influence the development of nursing services.Key points:We are a friendly and supportive team dedicated to providing the best possible care to our patients.We are a high performing practice, with high patient satisfaction rates.Competitive rates of pay.Circa 14,700 patients.Based in rural South Northants area with countryside views, with two sites close to the A5.6 weeks annual leave.
Main duties of the job Working autonomously, the ANP will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills.Key responsibilities:Assess and triage patients, including same day triage (working as part of our duty team) and as appropriate provide definitive treatment, including prescribing medications following policy, patient group directives, NICE guidelines and local care pathways, or make necessary referrals to other members of the primary care team.Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.A flexible approach is a must as we deliver face to face and telephone consultations as well as visiting housebound patients.
About us We have circa 14,700 patients cared for by an effective, dedicated, and comprehensive primary care healthcare team, which includes GPs, ACPs, Practice Nurses, Clinical Pharmacists, Social Prescribers, Podiatrist, First Contact Physio, Dispensary and Administrators/Receptionists.Part of Northants Rural PCN, we are a friendly and supportive team with an open and honest ethos, everybody is valued and opinions are heard.
Job description Job responsibilities Working within the NMC Code of Conduct, the post holder will deliver a high standard of care, making professionally autonomous and accountable decisions.
The post holder will work within their professional boundaries to demonstrate safe clinical decision-making and cost-effective, expert care for patients within the practice.
The post holder will support service development, the safe delivery of policy and procedures, and demonstrate effective nurse advocacy and leadership.
Assess / examine patients, diagnose, plan, treat and review / evaluate.
Assessment may be face-to-face by appointment or by telephone as part of a triage / duty care team and will include patients presenting with an undifferentiated diagnosis Diagnose and manage patients presenting with both acute and chronic conditions based on a management plan which includes a medication plan and other interventions.
Identify health priorities and make timely and appropriate interventions to support patients presenting in an emergency and/or with complex/urgent needs.
Assess and manage patients physiological and psychological needs, including patients with complex needs and / or multiple diagnosesMain Duties & Responsibilities:The following are the core responsibilities of the ANP / NP.
There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: Develop, implement, and embed health promotion and wellbeing programmes Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice Implement and evaluate individual specialised treatment plans for chronic disease patients Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF Provide wound care (ulcer/Doppler, etc.)
Provide specialist clinics such as ear syringing, contraception, etc.
Undertake the collection of pathological specimens Provide travel medicine services Request pathology services as necessary Process and interpret pathology and other test results as required Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required Maintain accurate clinical records in conjunction with extant legislation Ensure SNOMED/read codes are used effectively Maintain chronic disease registers Be responsible for the implementation of an effective immunisation programme Chaperone patients where necessary Prioritise health issues and intervene appropriately Support the practice team in dealing with clinical emergencies Recognise, assess, and refer patients presenting with mental health needs Implement vaccination programmes for adults and children Be an extended and supplementary prescriber, adhering to extant guidance Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required Contribute to practice targets (QOF, etc.
), complying with local and regional guidance Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)
Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual) Support the clinical team with all safeguarding matters in accordance with local and national policies Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately Deliver opportunistic health promotion where appropriate Act as the Infection Prevention and Control lead (or delegate accordingly) for the practice, ensuring compliance, undertaking audit as necessary Monitor and ensure the safe storage, rotation and disposal of medicaments Participate in local initiatives to enhance service delivery and patient care Support and participate in shared learning within the practice Continually review clinical practices, responding to national policies and initiatives where appropriate Participate in the review of significant and near-miss events applying a structured approach , root cause analysis (RCA) Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation Person Specification Qualifications Essential - Registered General Nurse holding current registration with the NMC - Non-medical prescribing qualification - Educated to masters level in a relevant field Desirable - Accredited training in chronic disease Experience Essential - Current experience of working in an independent and autonomous role, diagnosing and treating patients - Practice in improving and influencing the patient care pathway/journey - Demonstrates the ability to manage clinical risk and an understanding of health care governance systems - Experience of using clinical systems Desirable - Experience of audit - Familiar with SystmOne
