Job summary **PREVIOUS APPLICANTS NEED NOT APPLY**An exciting opportunity has arisen for the position of Clinical Pharmacist to join our welcoming and friendly team at Omnia Medical Practice, a forward thinking practice based in the East of Birmingham.We are looking to employ an experienced Clinical Pharmacist to join our existing team on a full time basis to help us expand and develop our chronic LTC management.The successful candidate will be joining a team of; 7 Partners, 3 Salaried GPs, 1 Pharmacist, 1 ANP, 3 Physician Associates, 3 Nurses and 4 HCAs.
Main duties of the job Main duties of the job The post holder is a pharmacy technician registered with the General Pharmaceutical Council.
They are required to act within their professional boundaries, supporting and working alongside a multi-disciplinary team within general practice and a Primary Care Network (PCN).
The post holder will contribute to improving prescribing practice, patient care and deliver the Directed Enhanced Service PCN prescribing agenda.
This includes improving repeat prescribing processes in General Practices within the PCN, promoting digitalisation, reducing medicines waste and minimising clinical risk associated with medicines taking.
The post holder will help patients to get the best from their medicines, operating within clear protocols and guidance.
They will be responsible for encouraging the development of better understanding of the principles of medicines optimisation throughout the practice.
The post holder will work with all sectors of pharmacy across the health system to improve population health.
Demonstrating improvement in patient outcomes and working to reduce health inequalities across the practice contributing to improving the efficiency of general practice and delivering a best in class service.

We work from a large modern purpose build facility offering a range of services caring for the needs of our patients which extends beyond traditional GP healthcare.
Our Community Hub, situated in reception was set up with our patients overall wellbeing in mind.
It offers a place to come together and enjoy shared interests and hobbies on a weekly basis with activities ranging from walking, knitting and gardening to bicycle tuition.We operate a nationally recognised social prescribing service and have high standing in our locality.The practice is made up of a large family of professionals comprising of GPs, Nurses, HCAs, TNA, a Pharmacist, Physiotherapist and our existing PA's.
In addition we have an experienced management and administration team who work together with the shared aim of providing excellent patient care to all of our patients.
This is a shared vision for both the partners and the staff of the practice.
Job description Job responsibilities The clinical pharmacist in a general practiceorganisation has the following key responsibilities in relation to deliveringhealth services.There may be, on occasion, a requirement tocarry out other tasks; this will be dependent upon factors such as workload andstaffing levels.A Clinical Pharmacist will:Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team Bea prescriber, or completing training to become a prescriber, and work with andalongside the general practice team Providespecialist expertise in the use of medicines whilst helping to address both thepublic health and social care needs of patients at the organisation and to helpin tackling inequalities Through structured medication reviews, supportpatients to take their medications to get the best from them, reduce waste andpromote self-care Havea leadership role in supporting further integration of general practice withthe wider healthcare teams (including community and hospital pharmacy) to helpimprove patient outcomes, ensure better access to healthcare and help managegeneral practice workload Developrelationships and work closely with other pharmacy professionals across thewider health and social care systemConsult patients within defined levels of competence and independentlyprescribe acute and repeat medicationReceive referrals and directed patients from triage services and othercliniciansReceive and resolve medicines queries from patients and other staffDeliver long term condition clinics and home visits, particularly for patientswith complicated medication regimes, and prescribe accordinglyReview medications for newly registered patientsImprove patient and carer understanding of confidence in and compliance withtheir medication To maintain accurate clinical records in conjunction with extant legislationEncouragecost-effective prescribing throughout the organisationOrganise and oversee the organisations medicines optimisation systems,including the repeat prescribing and medication review systems Secondary Responsibilities; Supportdelivery of QOF, incentive schemes, QIPP and other quality or costeffectiveness initiativesAgreeand review prescribing formularies and protocols, and monitor compliance levelsImprovethe data quality of medicines records and linking to conditionsSupportvirtual and remote models of consultation and support, includinge-consultations, remote medication review, and telehealth and telemedicineUndertakeany tasks consistent with the level of the post and the scope of the role,ensuring that work is delivered in a timely and effective manner Person Specification Qualifications Essential Be able to practice and prescribe safely and effectively in a primary care setting (for example, the CPPE clinical pharmacist training pathways) GPhC Registered Pharmacist Desirable Hold or be working towards an GPhC independent prescribing qualification Minor ailments certification Membership of the Royal Pharmaceutical Society Working towards faculty membership of the Royal Pharmaceutical Society Medicines management qualification MUR and repeat dispensing certification Experience Essential An appreciation of the nature of GPs and general practice.
An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
Experience and an awareness of common acute and chronic conditions that are likely to be seen in general practice An appreciation of the new NHS landscape, including the relationships between individual practices, PCNs and the commissioners Desirable Minimum of two years working as a pharmacist demonstrated within a practice portfolio Experience in managing pharmacy services in primary care In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare Understanding of the mentorship process Broad knowledge of general practice
