Job summary Do you have a passion or desire to work with people in the community?
Bury Town Integrated Neighbourhood Team (INT) are looking to recruit an ambitious Occupational Therapist into the team.
This is a rare opportunity to hold a permanent post within an established therapy team based in the community.
Working with colleagues from across health and social care you will be based at West Suffolk House and take an active part in the development and provision of therapy services as part of your role.
The successful candidate will work as part of the therapy team, being supported to develop to cover a variety of roles performed by colleagues in the team.
Using Occupational Therapy skills to assess and implement treatment plans for patients holding a variety of needs, the role provides the opportunity to have a direct impact on the people you will work with.
Increasing participation in meaningful activities in creative and dynamic ways with people in their own environments.
You will also have the opportunity to develop and grow within your Occupational Therapy role, learning new skills and developing knowledge relevant to your role within the community.

Main duties of the job As an Occupational therapist within the community Integrated Team you will have the opportunity to develop as an autonomous practitioner.
Utilising your skills as an Occupational therapist daily in peoples home to support patients working towards what matters to them.
You will promote and support people working towards independence in their homes and creatively problem solve with people to enable them to achieve their goals.
You will see patients daily with a variety of needs.
These will range from urgent visits to support patients with an acute need, work with patients who are in a period of reablement and work with those patients who have identified an increasing difficulty and would like to work towards preventing deterioration of performance of meaningful activities.
As a registered professional you will be required to manage your own diary and caseload ensuring patients have SMART goals and have appropriate therapy interventions.
You will be required to undertake regular continuous professional development appropriate to your role and scope of practice.
The community integrated team is a dynamic place to work this will require you to actively contribute to the development of the INT therapy service acute and wider alliance initiatives.
We operate a 7-day service with our current core hours being 8am to 6pm.

To manage a defined caseload, using evidence based/person centred principles to assess, plan, implement and evaluate therapy interventions.
To supervise band 3 and 4 staff.

Communication: To demonstrate effective communication skills with patients, carers, team members and people from other agencies.

Planning and organisational Skills: To work in collaboration with other members of the individuals support network in a complex multi-disciplinary, multi-agency environment.

To participate in the planning, development and review of the service.

To exercise good personal time management, punctuality and consistent, reliable attendance.

Clinical Duties: To effectively triage and schedule all therapy referrals to the service in accordance with Trusts policies and procedures.
To hold a caseload of patients with a range of multiple and complex health needs.
To utilise knowledge to carry out assessments, setting objectives and implementing appropriate treatment, as part of the holistic care plan for individuals and/or groups.
To formulate an individual management/treatment plan for each patient using wide range of assessment, clinical reasoning and treatment skills, including health coaching and promotion of self-management skills.
To ensure interventions are evaluated and plans modified to ensure effectiveness and timely discharge.
To provide training and advice to patients and carers to facilitate their care plan.
To adhere to agreed national, professional and Trust standards Person Specification Education and qualifications Essential oCurrent HCPC registration oDegree in Occupational Therapy.
oUp to date manual handling training for the lifting and manoeuvring of clients and equipment during treatment sessions Experience and knowledge Essential oCompleted clinical placements as a student oDocumented evidence of continuing professional development Desirable oClinical placement in a community setting oPost registration experience in a clinical setting oAdvice and support to students and support staff.
