Job summary The Millwood Partnership is looking for an enthusiastic Clinical Pharmacist Team Lead to join our pharmacy team.
Main duties of the job We are looking for an enthusiastic, highly motivated and experience Clinical Pharmacist to join our practice team.
You will effectively lead our pharmacy team to deliver patient-centred pharmaceutical care to our patients in General Practice.You will improve patients health outcomes and theefficiency of the primary care team by providing direct, accessible and timelymedicines and expertise including face to face contact with patients.
You will be an integral part of the generalpractice team, as well as part of a wider pharmacist network andmulti-disciplinary team.
This includes being a conduit of patient medicinesinformation into and out of hospitals.
You will work to optimise medication issues to improve patient care andsafety, and support clinical staff in the management of patients.
About us We have an exciting opportunity for a Clinical Pharmacist Team Lead to join our forward thinking practice.
We have 3 GP Partners, 5 Salaried GPs and we are looking to expand our team.
We are a total triage practice using Footfall for all of our appointment requests.
We have capped numbers per day and our clinical staff book their own appointments in line with their judgement of the patients condition.
You will be lead and develop our Pharmacy Team consisting of Pharmacy Technicians and Prescription Clerks, and our wider team of doctors and clinicians.
Job description Job responsibilities The clinical pharmacist in a general practiceorganisation has the following key responsibilities in relation to deliveringhealth services.
Workas part of a multi-disciplinary team in a patient-facing role to clinicallyassess and treat patients using their expert knowledge of medicines forspecific disease areas Bea prescriber and work with andalongside the general practice team Beresponsible for the care management of patients with chronic diseases andundertake clinical medication reviews to proactively manage people with complexpolypharmacy, especially the elderly, people in care homes, those with multipleco-morbidities (in particular frailty, COPD and asthma) and people withlearning disabilities or autism (through STOMP Stop Over MedicationProgramme) Providespecialist expertise in the use of medicines whilst helping to address both thepublic health and social care needs of patients at the organisation and to helpin tackling inequalities Provideleadership on person-centred medicines optimisation (including ensuringprescribers in the practice conserve antibiotics in line with localantimicrobial stewardship guidance) and quality improvement, whilstcontributing to the Quality and Outcomes Framework and enhanced services Throughstructured medication reviews, support patients to take their medications toget the best from them, reduce waste and promote self-care Have a leadership role in supporting furtherintegration of general practice with the wider healthcare teams (includingcommunity and hospital Pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage General Practice workload.
Developrelationships and work closely with other pharmacy professionals across thewider health and social care system Takea central role in the clinical aspects of shared care protocols, clinicalresearch with medicines, liaison with specialist pharmacists (including mentalhealth and reduction of inappropriate antipsychotic use in people with learningdifficulties), liaison with community pharmacists, and anticoagulation Toconsult patients within defined levels of competence and independently prescribeacute and repeat medication Toreceive referrals and directed patients from triage services and otherclinicians Toreceive and resolve medicines queries from patients and other staff Toprovide medication review services for patients in the practice and duringdomiciliary visits to the local nursing home Tomanage a caseload of complex patients Tomanage a therapeutic drug monitoring system and the recall of patients takinghigh risk drugs, , anticoagulants, anticonvulsants and DMARDs, etc.
Todeliver long term condition clinics and home visits, particularly for patientswith complicated medication regimes, and prescribe accordinglyToprovide pharmaceutical consultations to patients with long term conditions asan integral part of the multidisciplinary team Toreview medications for newly registered patients Toimprove patient and carer understanding of confidence in and compliance withtheir medication Tomaintain accurate clinical records in conjunction with extant legislation Toencourage cost-effective prescribing throughout the organisation Toimplement and embed a robust repeat prescribing system Toprovide advice and answer medication related queries from patients and staff Toorganise and oversee the organisations medicines optimisation systems,including the repeat prescribing and medication review systems Toimprove the quality and effectiveness of prescribing through clinical audit andeducation, to improve performance against NICE standards and clinical and prescribingguidance.
Todevelop yourself and the role through participation in clinical supervision,training and service redesign activities Toensure appropriate supervision of safe storage, rotation and disposal ofvaccines and drugs.
To apply infection-control measures within the practiceaccording to local and national guidelines Toprovide subject matter expertise on medication monitoring, implementing andembedding a system Tosupport clinicians with the management of patients suffering from drug andalcohol dependencies Toactively signpost patients to the correct healthcare professional Tomanage a caseload of complex patients and potential care institutions and toprovide advice for the GP management of more complex patients or areas such asaddictive behaviours, severe mental illness or end of life care Toreview the latest guidance, ensuring the organisation conforms to NICE, CQCetc.
Toprovide targeted support and proactive reviews for vulnerable, complex patientsand those at risk of admission and re-admission to secondary care Tohandle prescription queries and requests directly Toprovide proactive leadership on medicines and prescribing systems to theorganisation, patients and their carers Tosupport in the delivery of enhanced services and other service requirements onbehalf of the organisation Toparticipate in the management of patient complaints when requested to do so,and participate in the identification of any necessary learning brought aboutthrough clinical incidents and near-miss event To undertake all mandatory training andinduction programmes Tocontribute to and embrace the spectrum of clinical governance Toattend a formal appraisal with your manager at least every 12 months.
Once aperformance/training objective has been set, progress will be reviewed on aregular basis so that new objectives can be agreed Tocontribute to public health campaigns (, COVID-19 or flu clinics) throughadvice or direct care Tomaintain a clean, tidy, effective working area at all times.
Person Specification Qualifications Essential GPhC registered pharmacist An appreciation of the nature of Primary Care prescribing, concepts of rational prescribing and strategies for improving prescribing.
Experience and awareness of common acute and chronic conditions that are likely to be seen in General Practice An appreciation of the new NHS landscape, including the relationships between individual practices, PCNs and the commissioners.
Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face Excellent leadership, interpersonal, influencing, negotiation and organisational skills with the ability to constructively challenge the views and practices of managers and clinicians Knowledge of IT systems including ability to use word processing skills, emails and the internet to create simple plans and reports Ability to plan, manage, monitor, advise and renew general medicine optimisation issues in core areas for long term conditions Polite, clear telephone manner Ability to promote best practice regarding all pharmaceutical matters Effective time management (planning and organising) Demonstrate personal accountability and emotional resilience, and work well under pressure Desirable Experience of pharmacy services in primary care In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare Clinical system IT knowledge of SystmOne Personal Qualities Essential Ability to follow legal, ethical, professional and organisational policies/procedures and codes of conduct Ability to use own initiative, discretion and sensitivity Able to get along with people from all backgrounds and communities, respecting lifestyles and diversity Flexible and cooperative Ability to identify risk and assess/manage risk when working with individuals Sensitive and empathetic in distressing situations Problem solving and analytical skills Ability to maintain confidentiality Flexibility to work outside of core h ours DBS check Evidence of continuing professional development Desirable Full UK Driving Licence
