Job title: Support Practitioner | Care at Home ServicesResponsible to: Care at Home Service ManagerHours of work: Full-time (36.5 hours per week)Contract: PermanentLocation: Field-based working across all three localities of Aberdeen CitySalary: £22,Main responsibilities:Lead and participate in the provision of a high standard of person-led care and support for service users in their own homes, in line with individual care plans and risk assessments, organisational policies, national standards, codes of practice and legislation.Communicate effectively with service users, families and multi-disciplinary teams, record appropriate care notes and create a safe, positive and enabling environment.Job PurposeOur Care at Home team is looking for caring, compassionate and motivated people to join their growing team.

We will offer you extensive training to support your development in your role to enable a great sense of job satisfaction.Job ActivitiesTo understand the physical, emotional, cultural, and social needs of the service users, including a wide range of needs, illnesses and disabilitiesTo provide a wide range of domiciliary care including personal and continence care, managing medication, household tasks and meal preparation as agreed in individual care plansAn understanding of palliative and end-of-life careAn understanding of working with vulnerable elderly peopleWorking to a person-centred and enabling approachTo observe and promote the service user's choice, independence, dignity, privacy, fulfilment and other rightsTo actively support other Support PractitionersTo undertake the tasks detailed in the service user's care and support plan using trauma-informed practicesTo encourage the independence and motivation of the service user and foster improved levels of self-sufficiencyTo provide input into the care and support plans alongside the service userTo use manual handling equipment safely and correctlyTo take responsibility for the safe handling of property and equipment belonging to the service userTo establish and maintain good communication and develop effective working relationships with service users, families and multi-disciplinary teamsTo ensure as safe as possible the living environment for the service user, whilst respecting the service user's choice and rightsEssentialA minimum qualification of N/SVQ2 in Adult Health and Social Care, or equivalent professional qualification (nursing etc) (or equivalent professional experience and a willingness to work towards SVQ2)Literacy and numeracy at a level to maintain all the appropriate paperwork and report writing.Able to use a smartphone app and computer applications such as WordProven track record in a similar roleSystemsMaintenance of records – Utilisation of PASS to record all information in relation to Service User ManagementTeam meeting attendanceOffice and IT PlatformsQuality StandardsWorking to the appropriate Care Inspectorate standardsWorking to the appropriate Scottish Social Services Council standardsWorking to the appropriate Office of the Scottish Charity Regulator standardsWorking to the appropriate Aberdeen Cyrenians standards and working in partnership with the commissioner.Working to the appropriate Environmental Health standardsGeneral Terms of ReferenceIn carrying out the above activities, the Home Care Support Practitioner will:Work on their own and as part of teams, both within Aberdeen Cyrenians and with partnership organisationsMaximise their own performance and contributionEnsure they comply with internal and external regulations, procedures and other requirements pertaining to the serviceMaintain the good reputation of the organisationKeep abreast of developments and legislation relevant to the service's range of activitiesSeek to improve their knowledge and skills by both formal and informal meansCarry out other relevant work commensurate with the post as requiredAims, values and behaviours To be respectful of and work in accordance with the core values and behaviours of the organisation.To respect and work in accordance with our Equality and Diversity Policy and ensure its promotion in the course of your daily workHave a duty to take care of your own health and safety and that of others who may be affected by your actions at work.Co-operate with employers and co-workers to help everyone meet their legal requirements.Raise any specific queries or concerns you have regarding your workplace with your line manager or health and safety representative.
