Job summary Would you like to work as part of the hospice in home MDT and Hospice MDT.
We have various hours available which will be discussed at interview.You will take on the role as shift leader ensuring the shift runs smoothlyYou will be responsible for triaging calls over 24 hours and giving advice and support to patients who are known to St Helena or have their details recorded on the My Care Choices Register.You will also provide rapid response visits to patients in crisis in the community.The role will involve being a clinical resource to Staff nurses,and Health Care Assistants in the team, Administrators and Call Handlers within SinglePoint.You will work closely with community and primary care colleagues, St Helena and hospital staff in order to ensure that patients requiring end of life care receive an integrated service in a timely manner.The role will include working overnight, taking calls from patients and professionals and undertaking home visits as necessary throughout North East Essex.
Main duties of the job As a registered nurse/paramedic, providing high standards of holistic, individualised care and support to patients and families, through a process of assessment, planning, implementation and evaluation of care needs.To act as a resource to patients and their families in the community, providing telephone advice and support.To provide a rapid response visit to patients with urgent palliative or end of life care needs within the last 3 months of lifeTo be responsive to changing patient needs and flexibly visit according to suggested triaged response timeTo work shifts including overnight, triaging calls and visiting patients as necessary, often as a lone worker.Take responsibility for accurate assessments of patients and families and to provide ongoing plans of care for St Helena staff and other healthcare providers.To work with the Hospice in the Home Matron / Clinical Nurse Manager to plan and implement change and developmentTo provide supervision and work alongside Healthcare Assistants and provide guidance and expert advice, assisting in the identification and meeting of their educational needs.To ensure end of life procedures are carried out sensitively and appropriately.To verify death following training and when it is appropriate to do so in line with St Helena policy About us Here at StHelena, we believe that everyone living with an incurable illness has the rightto live and die with choice, compassion and dignity.
