Job summary Would you like to work as part of the hospice in home MDT and Hospice MDT.
We have various hours available which will be discussed at interview.You will take on the role as shift leader ensuring the shift runs smoothlyYou will be responsible for triaging calls over 24 hours and giving advice and support to patients who are known to St Helena or have their details recorded on the My Care Choices Register.You will also provide rapid response visits to patients in crisis in the community.The role will involve being a clinical resource to Staff nurses,and Health Care Assistants in the team, Administrators and Call Handlers within SinglePoint.You will work closely with community and primary care colleagues, St Helena and hospital staff in order to ensure that patients requiring end of life care receive an integrated service in a timely manner.The role will include working overnight, taking calls from patients and professionals and undertaking home visits as necessary throughout North East Essex.
