You will be pivotal in supporting and empowering the health promotion and health needs of our service user.Our service user all live in their own homes and have learning disabilities, some also have health health diagnosis in supporting the health needs and health promotion to support and empower people with a Learning disability.Each service user is staffed by a passionate and dedicated team.
This is an excellent opportunity to practice within the challenging yet rewarding field of community based learning disability services whilst working alongside a multi-disciplinary team.This Team leader post is a full-time Monday to Friday role.
We have an on call system in place to support the health care assistants in their roles.
The on Call system is 24 hours per day 365 days per year and is currently shared between the manager and the 3 team leaders.
This post will require you to become part of this on call rota.
This will have a up lift of salary, depending on how many you do and if called you will receive overtime payment for out of hours.
As this is a community post being a driver is essential for this role.
Main duties of the job This role requires you to demonstrate your commitment to work proactively and in partnership with people with learning disabilities and use your professional knowledge and experience to support individuals within the community and to maintain effective links with mainstream services, working in partnership with colleagues and supporting equal access to healthcare.To provide support and sometimes work directly with people with learning disabilities in their home, including working with their families, healthcare assistants, other health care professions and social workers.
You will be a dedicated Team leader for at least two services.To ensure all practice is based upon the principles of Valuing People; rights, independence, choice and inclusion.
To work with the individual, their carers in the formulation, implementation and review of a person-centred care support plan and risk assessments that will outline the support the individual requires to continue to live successfully within the community.As a Team Leader you will also support Health Care Assistants that work daily with our service users providing clinical expertise advice and guidance ensuring the specific needs of the service user are being delivered.
You will carrying out training and supervision, sickness return and sickness management, covering shifts via the rostering system in place, and help with medication management and support staff with various issues that are presents About us At Dorset HealthCare we're on a mission to empower people to make the most of their lives through our outstanding, joined-up healthcare services.
