Main duties of the job The team you will work with consists of an extensive MDTwhich includes nurse practitioners, occupational therapists, physiotherapists,health care assistants, therapy assistant/exercise practitioners andadministrative staff.
The team works closely alongside partner agenciesincluding those from the local and voluntary sector.
This is a significantstrength to our team we are proud of and supports patient centred, integratedworking.The team delivers planned and unplanned care to people intheir own homes, to assist in preventing hospital admission and supportingthose who have recently been discharged from hospital, require enhancedintervention and short-term rehabilitation support for their physical andfunctional health needs to remain at home.
The service also covers rotationinto Borrowdale House 25 bedded, reablement unit.
And Community Ready Unit a 19 bedded wardbased on the hospital site at South Tyneside.
The service is fast paced, and you would be expected to seea wide range of patients who present with a variety of conditions whensupporting the team clinically.

Job description Job responsibilities PLEASE REFER TO THE ATTACHED JOB DESCRIPTION FOR FULL DUTIES OF THE ROLE.To work across the services which provide VirtualWards, Urgent Community Response, Discharge to Assess and short termrehabilitation.To provide care to people in their own homes toprevent admission/ readmission to hospital by providing rapid assessment ofpatients in the community over seven days.Undertake comprehensive holistic assessment ofpatients in both planned and unplanned situations, be able to prioritiseclinical need, initiating investigations, determine a differential diagnosisand implement a plan of care reflecting evidence based guidance.To sustain agreed standards of care in to order tomaintain integrity and dignity of all patients in an environment which isconducive to delivering care in their usual place of residence or temporaryplace of residence including bed based rehabilitation.Undertake triage within the service determiningclinical priority of calls, working as a coordinator ensure teams aredispatched to respond to the referrals.Work collaborativelywith multi professional teams to ensure practice is efficient, effective andevidence based.To develop his /her scope of professional practicein order to support the team to meet service requirements in accordance withprofessional guidelines.To ensure the provision of expert knowledge to allusers of the service.
Person Specification Experience Essential Post qualification experience relevant to this role.
Experience of working with people with acute or chronic Long Term Condition.
Experience of multi-disciplinary working.

Desirable Participated in service developments.
Experience in participating in clinical audit.
Qualifications Essential Registered with the HCPC.
Degree.
Willingness to undertake a teaching qualification practice educator.
Desirable Leadership qualification.
