Band 7 - Children And Young People Transition Nurse Specialist

Job summary We are seeking a qualified, conscientious, and highly motivated professional to join the Paediatric Service at West Hertfordshire Teaching Hospital.This role offers an excellent opportunity for a senior nurse to take a leading role in advancing the care of patients transitioning from paediatric to adult services.
As a newly created position, there is significant potential to shape and develop the provision of care for this patient group.The post holder will collaborate closely with the Paediatric Team, Paediatric Consultants, Adult Medicine Team, MDT members, and both clinical and administrative colleagues across the Paediatric and Adult services.This is a full-time position, working hours per week (Monday to Friday).
Cross-site working may be required depending on service needs.
Main duties of the job West Hertfordshire NHS Teaching Hospitals Trust is developing a model of care to prepare and support young people in accessing an exemplary co-ordinated transition pathway between both Children and Adult Children and Young Peoples Transition Nurse Specialist will be an integral part of this developing service.
The role will include: Ensuring that the transition of care from children's to adult services is appropriately managed and delivered effectively within West Herts Facilitation of accurate, open communication and coordination with a range of people organisations, divisions, departments, and multi-professional stake holders.
Be a key member of the Children and Young Peoples team as well as supporting effective communication and stakeholder management.
Work collaboratively with the 'Carer for carers service'.
Ensure that the views of young people are intrinsic in the models of care developed.

Job description Job responsibilities Support service improvement by being able to identify areas for improvement around transition, support the development of any future business case.Support staff to be able to access relevant data and can then translate service data into meaningful performance information.Develop relationships alongside the CNS with staff outside of their area of service responsibility to deliver and improve services.Support staff to ensure patients have an up-to-date care plan that is being actively followed and updated.Promote the use of the hospital passport for young people with learning disabilities.Update, maintain, organise, gather, and analyse information to predict and meet future organisational and team needs by identifying best professional practice.
Person Specification Education and qualifications: Essential Registered Nurse (Child).
Registered Nurse (Adult) with Paediatric experience.
Educated to degree level in relevant subject or equivalent level of experience working at a similar level in specialist area.
Desirable Non-medical Nurse Prescriber.
MSc.
Knowledge: Essential Understanding of and commitment to equality of opportunity and good working relationships.
Understanding of the importance of planned and prepared transition pathway from children's to adult services.
Evidence of success in efficient and effective project and programme management.
Experience and understanding of evaluating and measuring the performance of health services.
Experience in communications and stakeholder management.
Good knowledge of project and/or health information systems.
Desirable A good understanding of the health and social care environment and roles and responsibilities within it.
Experience: Essential Experience of working with young people who have long-term medical conditions.
Can demonstrate experience in managing patients with multiple complex health conditions.
Experience with working with families and other multi-professional stakeholders, with experience of attending and contributing to meetings.
Used to working in a busy and often complex environment.
Experience of successfully leading and implementing service improvement projects.
Experience of working with young people with learning disabilities.
Ability to understand complex issues, problem solve and propose solutions.
Desirable Experience of working with young people with mental health problems.
