Job summary Home Care Practitioner - Eden ICCWorking along side the community team with a special interest in palliative care and rehabilitationThe team delivers care to patients which includes supporting with Activities of Daily Living, including personal care, catheter care, meal provision and support with nutrition.

Care should be delivered in such a way to promote independence where possible and as part of an ongoing plan to support the patient to remain in their own home.Care is delivered to ensure patients have a safe and timely discharge from hospital and prevent readmission or admission to hospital.
Main duties of the job Where possible to promote independence To deliver a high quality of personal care You will need to have excellent communication skills You will be part of a very motivated team -who deliver a high quality of care and rehabilitation The team works 7 days per week, 365 days per year and the shifts are 8 - 14:00 and then 15:00 - 21:00.
Staffs may be expected to change their shifts pattern and work outside these hours with limited notice.
