You must have the ability to prioritise your working day under pressure, whilst ensuring the values and behaviours of the Trust are upheld.We encourage and welcome applicants to carry out an informal visit to our unit and we are happy to answer any questions you may have Main duties of the job An experienced health care professional competent in all aspects of care planning, who is able to carry out a wide range of clinical skills, including area(s) of specialist clinical expertise and advice within the team infection control, by working collaboratively and co-operatively with others to meet the needs of patients and their families.The post holder will also take an active part in the leadership, education, development and supervision of the team.
About us MCHFT provides a full range of local hospital and community services for people across East Cheshire (population 399K) and West Cheshire and Chester boroughs (population 357K).The Trust provides high quality planned and unplanned/emergency care, cardiac and critical care as well as child health, maternity services, and intermediate care across our sites at Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford.The Trust provides 500 beds and employs over 5,000 members of staff.An extensive range of community services is provided across 26 medical centres and schools by our community arm, Central Cheshire Integrated Care Partnership, in collaboration with Cheshire and Wirral Partnership NHS FT and the South Cheshire and Vale Royal GP Alliance.

Job description Job responsibilities Perform comprehensive assessment of patient nursing needs, plan, implement and evaluate care according to changing health care needs, including providing clinical expertise, advice and guidance in a specialist area of clinical practice to improve the overall care provided by the teamWork collaboratively within the multidisciplinary team/agencies to ensure that patient needs are metInvolve patients, relatives/carers in the delivery of care, providing reassurance and support to assist their understanding and co-operationPrepare the clinical area for safe practice by ensuring that cleanliness is maintained, materials and equipment are ready for use, including clearing away afterwardsProvide escort and transfer for patients as directed by the multidisciplinary teamRecognise and respond appropriately to urgent and emergency situationsCollect, collate, and report information, maintaining accurate patient records/documentation Person Specification Knowledge Essential Evidence of knowledge within field Desirable Detailed knowledge of professional accountability and NHS issues Experience Essential Years of experience Desirable Previous Acute Medicine Experience Communication Skills Essential Ability to respond to different needs of patients in different settings Desirable Evidence of contribution to meetings/discussions outside of team
