Job summary The trainee practitioner will work and be supported by one of our qualified frailty practitioners.
They will undergo a 2 year training programme, to develop their skills and knowledge to support their practice within our care of the Elderly/frailty services across the trust.
This training will involve and advanced clinical skills course and the non medical prescribing module at masters level, as well as a clinical competency portfolio.
The post will be based at Northumbria Specialist Emergency care Hospital, but their will be rotation across our other services including: Acute frailty service - NSECH Elderly Assessment Unit - WGH Frailty Virtual ward - Community Community rehabilitation wards (Alnwick, Whalton Unit & Blyth) Please note we reserve the right to close this vacancy prior to the closing date once the required number of suitable applications have been received Annex 20 This post falls within the provisions of Agenda for Change Annex 20 - Development of Professional Roles, whereby upon completion of an agreed set of competencies you may move onto the next pay band without the need for an application for a post at a higher level.
For further information on Annex 20, please see: Main duties of the job To develop the skills required to provide advanced clinical assessment of patientsTo triage patients, prioritising clinical need, highlighting and initiating treatment plans by following agreed Trust guidelines and protocols whilst under supervisionTo develop the skills to work autonomously, assessing individual patient needs, initiating investigations, determining a differential diagnosis and initiating appropriate holistic, evidence based treatment and careTo provide senior nursing support on operational issuesTo role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the trust.
This means demonstrating a consistent leadership style which (a) engages, enables and empowers others (b) uses coaching to promote ownership of learning and quality improvement and (c) facilitates team working and collaboration within teams / departments and across organisational boundaries.

Job description Job responsibilities The trainee practitioner will work as part of the wider frailty team with in Northumbria.
They will with support conduct comprehensive geriatric assessment on patients in a variety of settings.
This will include: Clinical history taking Clinical examination Requesting investigations Interpreting investigation results initiating treatments including prescribing (once V300 obtained) Discussing treatment escalation and health carte plans with patients and their relatives.
Identifying right place first time for patients to receive the best possible care for their ongoing needs.
Documenting individualised care plans for their patients.
All of this will be done whilst working within the larger practitioner workforce and other healthcare professionals as part of the wider MDT.
We welcome individuals who are passionate about frailty/COTE with an interest to progress their careers within an enhanced/advanced clinical role within this speciality.
It would be desirable for applicants to have some experience/knowledge of working within this frailty/COTE.
Individuals must be willing to undertake advanced clinical skills training and non-medical prescribing at masters level.
This training will be built into a bespoke training programme over a 2 year period.
On completion of the required academic training and clinical competency portfolio the trainee will promote to a Band 7 autonomous frailty practitioner within the team.
Person Specification Qualifications Essential 1st Level NMC registered Nurse or Allied healthcare professional registered mentor degree level study with evidence of ongoing CDP ENB998/730 or equivalent Desirable Advanced clinical skills qualification ILS/ALS IRMER course Minor Injuries qualification/experience Nurse Practitioner qualification (NACAN Masters Level) Other Essential It is an essential requirement of the role that the post holder has a valid driving licence and is either a car owner and able to use the car for work purposes, or has a Trust personal lease vehicle which may be used for the role.
