We care for patients in their home using the virtual ward, and through the whole span of their journey within the acute hospital - from specialist assessment in the emergency department to seamless specialist care in our frailty assessment base.

Main duties of the job The successful candidate will run ward rounds on our virtual ward supported by a Staff Grade doctor or SpR.
The post holder may act as the clinical supervisor for the junior team.
This will involve a daily consultant led board round on an online platform (TEAMS).
The post holder will be expected to provide appropriate clinical leadership to the progressive, innovative multidisciplinary ward team.
The post holder will have scheduled time working in the Emergency Department as part of the FIT team, completing frailty assessments of patients at the 'front door'.
These patients can then be seen in FAB and the successful candidate will be required to cover 2 sessions per week whilst listed on the time table below the actual session will vary from week to week as cross cover is included in the PA calculation.
This role would support REACT services with clinical expertise they link in with the community Dementia Intensive Support Team, social services and the Red Cross to support people in through a short term deterioration in health and function.
During the time in FAB/ front door frailty you may also hold the "hot phone" this is a mobile phone which enable any GP or community therapist to phone for advice or to book a same/next day patient into FAB.
Our FIT team also use this for support and some appointments each day are reserved for direct transfers from ED for patients who can probably be discharged but need a more comprehensive/specialist assessment before leaving.

Job description Job responsibilities This post can include a slot on the general medicine on call rota.
This is 1 in 8 weekends 1 day only, there is a 12 hour shift in EAU reviewing patients admitted acutely, then availability overnight from home if needed.
This is supported by an acute on call junior team and 2 medical SpRs during the day with a Hospital at Night scheme overnight.
The successful candidate will have the option of joining this rota if they wish.
The GLINT (Geriatric Liaison to the Integrated Neighbourhood Teams) is our plan to move some falls assessments out of FAB and into the local teams with consultant support this will improve patient experience and also stop duplication of assessment between community and FAB therapists.
This clinic is established in some localities and being developed in others.
We invite those candidates with sub-specialist skills and attributes, for example, onco-geriatrics, orthopaedic liaison, or virtual ward community experience, and we would be happy, where possible to negotiate time within the timetable to allow these to flourish.
Person Specification Essential Essential Inclusion on the GMC Specialist Register for Geriatrics or within six months of expected date of CCT(or be eligible for entry onto these registers within 6 months of application) MRCP or equivalent Evidence of working in a multi-disciplinary team Ability to organise and prioritise complex demands Ability to offer expert clinical opinion on a full range of acute medical emergencies, rehabilitation and intermediate care Evidence of teaching and training skills for junior doctors Ability to manage risk and end of line decisions Full Driving Licence and regular access to a car Desirable Training in appraisal techniques Postgraduate diploma or higher degree Knowledge of current national agendas affecting the NHS and the implications involved Evidence of having undertaken aufit work that has brought about change Management training and experience Training in teaching human factors training Live within a 10-mile radius of the relevant hospital site
