Job summary I G Medical are looking for their new Lead Practice Nurse to join our team based in Ilkley with some travel to Grassington.This position will be replacing our current lead nurse who steps down next year.The role is key to supporting and developing our nursing and healthcare team, working alongside a senior management team and partners.

Main duties of the job The LeadPractice Nurse will be responsible for the management of the practice nursingteam, consisting of nurses and HCAs across two sites.The LeadPractice Nurse will be responsible for several clinical areas such as infection preventionand control, health promotion, chronic disease management, health prevention,well women and well man clinics, as well as supporting the practice management team in the reviewing and delivery of clinicalpolicy and procedure.
The LeadPractice Nurse will be responsible for the implementation of processes for theeffective management of patients with long-term conditions using evidence-basedpractice including care for elderly and housebound patients.
Working asa senior member of the practice multidisciplinary team, the post holder willensure nursing services are delivered effectively to the entitled patientpopulation.
About us IG Medical is situated in West Yorkshire with sites in Ilkley and Grassington.
Our patient list is approximately 18,000.Our clinical team includes 5 GP Partners, 11 salaried GP's, 2 Advanced Clinical Practitioner and our nursing team consists of Practice Nurses and Health Care Assistants.

There may be, on occasion, a requirement to carry outother tasks.
This will be dependent upon factors such as workload and staffinglevels:a.Toact as the Infection Prevention Control (IPC) lead and to work to the IPCPolicy providing audits, training, support and evidence of compliance to thepractice manager.b.Toproduce the IPC annual statement in conjunction with the practice managerc.Todevelop, implement and embed an effective care management programme for thefrail and housebound patients registered at the practiced.Toassess the needs of patients ensuring the provision of healthcare isappropriate, incorporating evidence-based practicee.Todevelop, implement and embed health promotion and wellbeing programmesf.Toimplement and evaluate individual treatment plans for chronic disease patientsthat promote health and wellbeingg.Toidentify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients healthh.Asrequired, to provide routine nursing care to patients in accordance withclinical based evidence, NICE and the NSFi.Asrequired, to diagnose and manage acute and chronic conditionsj.Toprescribe medication for patients in accordance with PGDs and own scope ofpracticek.Toreview patient medications to enhance compliancel.Toprovide guidance to patients on the use of prescribed and over-the-countermedications regarding side effects and dosagesm.Toco-ordinate the provision of travel medicine servicesn.Torequest pathology services and process pathology results as requiredo.Tooversee the provision of chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as requiredp.Tomaintain accurate clinical records in conjunction with extant legislationq.Toensure snow med codes are used effectively by all members of the nursing teamr.Toensure the appropriate maintenance and use of chronic disease registerss.Toensure compliance with the practice chaperone policyt.Toassist GPs with minor surgery when requiredu.Toprioritise health issues and intervene appropriatelyv.Tosupport the team in dealing with clinical emergenciesw.Torecognise, assess and refer patients presenting with mental health needsx.Toimplement vaccination programmes for adults and childreny.Tosupport patients in the use of their prescribed medicines or over-the-countermedicines (within own scope of practice)z.Toliaise with external services/agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)aa.

To understand practice and localpolicies for substance abuse and addictive behaviour, referring patientsappropriatelydd.Todeliver opportunistic health promotion where appropriate Person Specification Qualifications Essential Registered Nurse (Nursing and Midwifery Council) Completed or working towards formal infection prevention and control training Nurse prescriber Desirable Mentor or teaching qualification Post graduate diploma or degree (Chronic Disease Management) Family planning qualification Clinical Knowledge and Skills Essential Wound care/removal of sutures and staples ECGs Venepuncture New patient medicals Requesting pathology tests and processing the results, advising patients accordingly Understanding the importance of evidence-based practice Understand the requirement for PGDs and associated policy Ability to record accurate clinical notes Ability to work within own scope of practice and understanding when to refer to GPs Chaperone procedure Womens health (cervical cytology, contraception, etc.)
