Healthcare Support Worker- Palliative Care

Job summary The post holder will be a member of the Hospice at Home Team working under the guidance of the Team Leader.
She/he will provide basic nursing / social care and support to patients and their families, who are in the last weeks of life, in their own homes.The post holder will work within the Hospice at Home team for the Betsi Cadwaladr University Health Board, in partnership with Community Services, Community Hospital, Local Health Boards and the charitable and independent sectors.The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job To work together with, and compliment, the existing multi-agency community care team, in extending the available care to patients and their families/carers, who are in the last weeks of life at home.To work autonomously and unsupervised in the community, under the direction of the Team LeaderTo assess the individual needs of the patient and their families / carers.To provide basic nursing care to patients, personal care, catheter care, minor dressings, stoma care, and offer respite support to their families / carers, according to QCF / NVQ competencies.To provide social care to patients, this may include minimal meal preparation and nutritional support.To provide personal care to deceased patients as appropriate and according to the wishes of the deceased's family / carers.To provide holistic care to patients and their families / carers, being ready to meet searching questions on a personal level, demonstrating effective communication skills.To demonstrate adaptability and responsibility when dealing with unpredictable situations, support to distressed patients and their families / carers and contacting appropriate healthcare professionals / agencies in emergency situations.To undertake other specialised procedures, according to the patient's need, appropriateness and the health care support worker's competencies.
