Job summary We are delighted to offer this exciting opportunity to join our Integrated Out of Hours (IOOH) Team as a Band 6 NMC Registered Senior Triage Nurse.
We are looking for a passion and dedicated senior nurse to provide daily supervision and leadership to the IOOH LAP team including provision of expert advice to call handlers and triage nurses to support clinical decision making.
You may also be expected to conduct some priority nursing visits where referrals at point of triage are deemed as an essential visit.
Following your findings you would then pass the information over to the Community Nursing Teams to continue with the patients care, therefore advanced skills in community nursing will be essential.
You will represent the IOOH team at capacity calls maintaining effective relationships with the community nursing teams; ambulatory clinic services and the out of hours community nursing.
Keeping up to date with, and sharing any changes that may impact the LAP team.
You will have responsibility for ensuring appropriate cover and skill mix in the IOOH team to provide effective triage between to 7 days a week.
Ensuring there is a seamless interface with the service provided from 8am to Main duties of the job Your role will require you to determine eligibility of patients referred to the service and collect additional information to support decision making.
To transfer or redirect referrals to appropriate internal or external services as appropriate.
As part of this role you will also be expected to provide: Plans of care for the ongoing management of patients who require visits from the community nursing team for a wide range of nursing care.
A review all referrals and messages to the community nursing service, this will include an understanding of the following areas of care: Symptom management of deteriorating patients.
Palliative and end of life care.
Wound care.
Bladder catheter and bowel care.
Recognition of deteriorating patients and potentially life threatening conditions.
Complex patients with co-morbidities.
Increase functional ability.
Improvement in physical and mental health and well-being.
Interventions in disease and condition management.
Admission avoidance.
Supporting discharge.
You will work closely with the operational lead to ensure that all members of the IOOH LAP team have access to monthly supervision; annual appraisal and review and support with personal career development planning.
Work with the team BSO to complete rota planning for the IOOH LAP team to ensure cover for the service including annual leave and cover for sickness.
