Job summary Invicta Health Community Interest Company are delighted to be recruiting for a Care Co-ordinator, across our three GP practices in Romney Marsh, New Romney and Lydd.
Main duties of the job As a Care Co-Ordinator, you will provide animportant role in supporting the smooth running of clinics by performingroutine administration tasks, combined in some areas with basic clinical taskson behalf of the GP, allowing them to spend their time focussing on appointmentsand improving patient experience in the surgery.
As a Care Homes Care Co-Ordinator, youwill be responsible for helping to improve the continuity of care by acting asa point of contact for residents, families and professionals who visit, or workin the care home, such as multi-disciplinary team (MDT) members and in-reachspecialists.
You will work to provide extra time, capacity and expertise tosupport individuals in preparing for, or in following- up clinicalconversations they have with primary care professionals.
You will work closelywith primary care professionals within the PCN to identify and manage acaseload of identified care homes; making sure that appropriate support is madeavailable to the staff and their residents; and ensuring that their changingneeds are addressed.
You will focus delivery of the comprehensive model toreflect local priorities, health inequalities or population health.
About us Invicta Health was formed in 2008 with the aim of supporting primary care services in the local area.We currently operate a number of General Practices within the Romney Marsh, Dover, Birchington, Broadstairs and Ramsgate areas.
Invicta Health manages other services including the Primary Care Mental Health Specialist service which is delivered across east Kent and Swale.
As part of a multi organisation alliance, Invicta Health also provide General Practitioners and clinical support for the Urgent Treatment Centres based in Kent & Canterbury Hospital and Buckland Hospital (Dover).Invicta Health is a Community Interest Company limited by shares.
This means that the company has not been established or conducted to make private gain.
The majority of any profit made needs to be available for investment back into the company or for the benefit of the community.We are a shareholding company and can only distribute a small proportion of this to the shareholders and this will only happen if the organisation is confident that doing so will not impact on the care provided.
We currently support a number of local community services and invest into our services and sites on a regular basis.We have developed a number of partnerships ranging from our local acute trust, community and mental health trust, local federations, out of hours providers, practices and third sector organisations.
Job description Job responsibilities Main Responsibilities Sorting all clinical postand prioritising for the GP in terms of actions.
Signposting some post toothers such as clinical pharmacist where necessary.Extracting all informationfrom clinical letters that need coding and summarising and action accordingly.Arranging appointments,referrals, tests and follow up appointments of patients.Identify triggers and urgentsituations and refer to the correct healthcare professional as appropriate.Completing basic forms and core elements of some forms for the GP to approve andsign such as insurance forms, mortgage, benefits agency forms etc.Explaining treatmentprocedures to patients, including arranging follow up appointments.Helping the GP liaise withoutside agencies, for example getting an on-call doctor on the phone to askadvice or arrange admission while the GP can continue with theirconsultation(s).Work within amulti-disciplinary team, providing support inarranging and making adjustments to work tasks and schedules as necessary.To support the practice withQOF and associated claims.Monitor clinical and organisational targets, reviewing the practicesperformance against QOF compliance on a monthly, quarterly, and annual basis,liaising with the Practice Manager to address any areas of potential or actualunder performance.Identify and claim allavailable income for the practice through the various claims systems accuratelyand in adherence to deadlines.Monitor APEX, CQRS and PCQS claims monthly, working to identify trendsand investigate any anomalies.
Reporting to the management team whenrequested.Produce accurate monthlyreports on practice QOF and Enhanced Services (ES) claims and present these at managementmeetings when requested.Conduct searches and reports as required using the EMIS system.Proactively identify and work with a group ofcare homes to support personalised care requirements for their residents, usingthe available decision support aids.
Help people to manage their needs, answeringtheir queries and supporting them to make referrals into the Care Home MDT orescalation of need to the Care Home Clinical Team.Provide coordination and navigation for peopleand their carers across health and care services, working closely with socialprescribing link workers, health and wellbeing coaches and other primary careroles.
Lead the coordination and delivery of the CareHome MDTs within the PCN.
Person Specification Qualifications Essential Educated to A Level or equivalent or relevant work experience Desirable Higher education certificate Experience Essential Knowledge and understanding of different claim categories and processes within primary care Previous QOF experience Understand the importance of maintaining patient and data confidentiality.
Experience of working with Microsoft packages including, word, excel and power point.
Knowledge of read codes/SNOMED codes.
Desirable Previous experience of processing complex requests Knowledge and experience in the use of the E-referral system (ERS) Experience of working with EMIS and DOCMAN Knowledge and understanding of the National Health Service
