HOURS: Full time (Permanent)
RESPONSIBLE TO: Team Leader
Salary: £27,730 - £27,980 (inclusive of 2 sleeps per week at £55 per sleep)
Shift patterns role (Rota shifts- 2 sleep ins required per week*)

MAIN JOB PURPOSE: We support the people we work with to lead a life worth living. Project Workers provide direct one to one support to the people we work with, both in their homes and in the community. Project Workers support the people we work with in all aspects of their daily lives, working in a person-centred way to ensure they have as full and independent life as possible. The role can be challenging but is also very rewarding.

SPECIFIC TASKS: The needs of the people we support are very varied and, as a result
so is the work Project Workers do. The role involves but is not limited to supporting
people with maintaining their home, doing day to day tasks such as shopping and
attending medical appointments as well as leisure activities such as fishing, bike rides,
cinema trips. Each person we support has a Support Plan for project workers to follow.
Support plans are developed in partnership with the people we support and are tailored
to their needs and wishes.

WORK WITH INDIVIDUAL PEOPLE WE SUPPORT
Each person will have different support needs, interests, wishes and personality. As a
Project Worker you will be supporting someone to lead as full and independent life a life
as possible. You will be supported by us to get to know the people we work
with and build safe and supportive relationships with them. You will have the opportunity
to meet the people you will be supporting, get to know them and learn how best to
support them before working alone.

Once you are ready to lone work you will encourage and support the people we work
with to; maintain pride in their homes, look after their physical and emotional health, find
and attend work or educational opportunities, develop hobbies and interests, maintain a
positive community presence and keep themselves and others safe.
You may be asked to ‘key work’ someone you support in which you offer support to a
specific individual in a variety of things such as; managing their tenancy, coordinating
health appointments, supporting in financial management/planning, maintaining
documentation such as day note and finance summaries and planning activities such as
family contact and arranging trips out and holidays. Once you are established you will
be expected to support new staff to learn the role and develop relationships with the
people we support.

WORK WITH THE COMMUNITY
Supporting those you are working with to take an active role within the local community
by being a valued community member/neighbour and positively contributing within their
local area. This will require you to represent the person you support, the organisation
and care sector with pride and a sense of responsibility.

REPRESENTING SOCIAL CARE PROFESSIONALLY AND PROFICIENTLY
We provide 24-hour support to the people we work with. Therefore, there is a
requirement for staff to undertake evening and sleepover shifts. To work alternate
weekends and contribute to cover over the Christmas period.
Offer support to other staff members and receiving support from them.
Participating in and preparing for staff team meetings, supervision, staff training and
organisational meetings.
Having a good value base, being aware of the rights of others, understanding your duty
of care and duty of candour.
Communicating with and contributing positively to the wider staff team.
Always maintaining confidentiality regarding the people we support and the wider
organisation.
Reporting any incidents, accidents or complaints using the relevant policy guidance.
Ensure you are familiar with and adhere to policies and practices.

ADMINISTRATIVE REQUIREMENTS
Read and follow diary and communication book entries, making clear entries when
required.
Reading and completing daily notes accurately, clearly and professionally.
Completing finance account book entries clearly and accurately.
Completing and submitting incident and accident forms as appropriate.
Completing holiday forms and mileage forms within the timeframes specified within the
organisational policy
