The Role
Position: Band 6 Support Worker
Location: West London
Duration: 3 to 6 months (with potential extension)
Schedule: Monday to Friday (9:00 am - 5:00 pm)
Start Date: ASAP
Pay Rate: £30 p/h
Your Daily Impact
As a Band 6 Support Worker, you’ll play a vital role in empowering individuals with complex needs to live fulfilling lives. Your day-to-day responsibilities will include:
Supporting individuals with physical, mental health, and social care needs, enabling them to achieve their goals.
Collaborating with a multidisciplinary team to ensure seamless care and service delivery.
Conducting assessments and creating tailored support plans that promote independence and well-being.
Providing guidance on harm reduction, health promotion, and relapse prevention strategies.
Acting as a key contact for service users transitioning between services, ensuring smooth handovers and continuity of care.
Responding promptly to safeguarding concerns with professionalism and care.
Maintaining clear, accurate records that meet organizational and professional standards.
What You Bring
Qualifications:
Social work experience essential.
May consider a nurse with substantial support worker/social worker experience.
Experience: A strong background in health, social care, or substance misuse services is desirable.
Skills: Exceptional communication, teamwork, and problem-solving abilities.
