We are seeking an experienced Band 6 Social Worker to join our team in West London. As a vital member of our multidisciplinary team, you will play a key role in empowering individuals with complex needs to live fulfilling lives.
The successful candidate will have a strong background in health, social care, or substance misuse services and possess exceptional communication, teamwork, and problem-solving abilities. A compassionate, person-centered approach is essential for this role, as you will be working closely with service users to create tailored support plans that promote independence and well-being.
About the Role
This is a 3-6 month ongoing contract with potential extension. The ideal candidate will work Monday to Friday (9:00 am - 5:00 pm) and enjoy flexible shifts tailored to their lifestyle. Your Daily Impact
As a Band 6 Support Worker, you will be responsible for:
- Supporting individuals with physical, mental health, and social care needs, enabling them to achieve their goals.
- Collaborating with a multidisciplinary team to ensure seamless care and service delivery.
- Conducting assessments and creating tailored support plans that promote independence and well-being.
- Providing guidance on harm reduction, health promotion, and relapse prevention strategies.
- Acting as a key contact for service users transitioning between services, ensuring smooth handovers and continuity of care.

Salary and Benefits
