Sefton Council are seeking 3 dedicated and experienced Social Workers to join their Learning Disability & Autism Team within Adult Social Care. Responsibilities:
As part of the Learning Disability & Autism Team, your key responsibilities will include:

Transition Management: Overseeing the current transition allocation list, ensuring timely assessments and smooth transitions for young people entering adult services.
Case Management: Managing a caseload of complex cases, providing assessments, and managing risks effectively.
Timely Interventions: Addressing any backlog of referrals and ensuring that assessments are carried out in a timely and efficient manner.
Collaboration: Working closely with other professionals, families, and carers to develop person-centred care plans and ensure holistic support for individuals.
Teaching and Support: Supporting the teaching and assessment of social work students during their placements and offering guidance to less experienced staff members.
Court Proceedings: Providing evidence in court as required, particularly in relation to care proceedings.Requirements:

Have substantial experience in adult social care, particularly within learning disabilities and autism services.
Hold a degree in Social Work and are registered with Social Work England.
Enhanced DBS.
