Service Care Solutions is seeking a dedicated and experienced Social Worker to join their Learning Disability & Autism Team within Adult Social Care. This role offers the opportunity to work closely with young people transitioning into adult services, ensuring timely assessments and support during this critical period.

Key Responsibilities:
  • Transition Management: Oversee the current transition allocation list, ensuring timely assessments and smooth transitions for young people entering adult services.
  • Case Management: Manage a caseload of complex cases, providing assessments, and managing risks effectively.
  • Timely Interventions: Address any backlog of referrals and ensure that assessments are carried out in a timely and efficient manner.
  • Collaboration: Work closely with other professionals, families, and carers to develop person-centred care plans and ensure holistic support for individuals.
  • Teaching and Support: Support the teaching and assessment of social work students during their placements and offer guidance to less experienced staff members.
  • Court Proceedings: Provide evidence in court as required.
