Job Summary :

We are seeking a compassionate and dedicated Social Care Assessor to provide essential support to individuals in need of assistance. This role is integral to ensuring individuals within the community receive the support they need to thrive. You'll also consider D2A to determine the person's care and support requirements. Create and agree on a personalised care plan while managing expectations.


Duties :

  • Assist clients with daily living activities, including personal hygiene, meal preparation, and medication management.
  • Provide companionship and emotional support to clients, fostering a positive environment.
  • Support clients with mobility and transportation needs, including driving them to appointments or social outings.
  • Engage in behaviour management strategies to assist clients with specific needs.
  • Collaborate with healthcare professionals and family members to develop and implement personalised care plans.
  • Document client progress and report any changes in condition or behaviour promptly.


Experience :

  • Strong commitment to enhancing the quality of life for clients, particularly in areas such as senior care, childcare, and patient care.
  • Proactive approach to care, ensuring that clients receive the necessary support while promoting their independence and dignity.
  • Previous experience in hospital settings or assisted living environments is highly desirable.
  • A valid driving license is preferred to facilitate transportation of clients as needed.
  • Strong interpersonal skills and the ability to communicate effectively with clients, families, and colleagues are crucial for success in this role.
