As a key member of our Wiltshire Support at Home team, you will play a crucial role in providing short-term homecare support to promote independence and prevent, reduce, and delay the necessity for long-term care.

Key Responsibilities
  • Provide high-quality care and support to individuals in their own homes, promoting independence and dignity.
  • Work collaboratively with the Rapid Response Service to provide short-term support and prevent hospital admissions.
  • Develop and maintain strong relationships with service users, their families, and other healthcare professionals.
  • Contribute to the development and implementation of care plans, ensuring that individual needs are met and exceeded.
  • Participate in ongoing training and professional development to enhance skills and knowledge.
What We Offer
  • A competitive salary of £23,893 - £24,702 per annum.
  • A comprehensive training program, including diploma opportunities and career progression.
  • A welcome payment of £1300 (subject to tax and NI) for your integral role in reducing flow pressure on hospitals.
