Job Summary

We are seeking a highly skilled and experienced Advanced Clinical Practitioner to join our team at Oxleas NHS Foundation Trust. As a key member of our primary care team, you will be responsible for providing high-quality healthcare services to patients in a secure environment.

Key Responsibilities
  • Exercise personal and professional autonomy to make complex decisions and judgments.
  • Lead the development and implementation of innovative healthcare services, including long-term condition management and health promotion activities.
  • Provide expert clinical leadership to the primary care team, ensuring a proactive approach to quality, trauma-informed, and outcome-driven services.
  • Develop and participate in managerial and professional clinical supervision programs to ensure staff receive appropriate supervision, training, and annual appraisal.
  • Implement effective patient-centered care planning tools and a trauma-informed model of delivery and integrated services within the establishment.
  • Work collaboratively with other clinical leads, GPs, psychiatrists, and clinicians to maintain and review service needs as required.
  • Undertake ongoing training to continually improve primary care provision.
Requirements
  • Registered RGN with a postgraduate ACP qualification at degree level or equivalent.
  • Experience in middle management, delivering primary care services in a multidisciplinary team.
  • Experience of managing staff, service development, implementation, and change management.
  • Knowledge of models of care for primary care within secure environments and current issues/agendas facing prison healthcare.
