We are seeking an experienced and motivated Advanced Clinical Practitioner to join our team at Oxleas NHS Foundation Trust. As a key member of our primary care team, you will be responsible for providing high-quality healthcare services to patients in a prison setting.
About the RoleThis is an exciting opportunity to work in a dynamic and challenging environment, where you will have the chance to develop your skills and expertise as a clinical leader. You will be responsible for providing expert clinical leadership to the primary care team, ensuring the delivery of high-quality, patient-centered care.
Key Responsibilities- Provide expert clinical leadership to the primary care team, ensuring the delivery of high-quality, patient-centered care.
- Develop and implement innovative practice, leading to improved patient outcomes and experiences.
- Work collaboratively with other clinical leads, GPs, and other clinicians to maintain and review service needs.
- Ensure the development of evidence-based practice in this specialist field, promoting research and quality improvement initiatives.
- Implement effective patient-centered care planning tools and a trauma-informed model of delivery and integrated services within the establishment.
To be successful in this role, you will need to have:
- Registered RGN qualification.
- Non-Medical Prescribing qualification.
- Evidence or experience of Leadership/Management qualification.
- Postgraduate ACP qualification at degree level or equivalent.
- Middle management experience in a MDT delivering primary care services.
- Experience of managing staff.
- Experience of service development, implementation, and change management.
- Evidence of working as a nurse prescriber.
