Key Responsibilities:
- To provide person-centred support and care to individuals, promoting their independence and dignity.
- To work collaboratively with colleagues and external partners to develop and implement individualised care plans.
- To maintain accurate records and reports, ensuring compliance with regulatory requirements.
Requirements:
- A clean driving licence (essential).
- Ability to work flexibly, including evenings and weekends.
- Excellent communication and interpersonal skills.
