We are seeking a highly motivated and compassionate Health and Wellness Assistant to join our care team. As a valued member of our team, you will play a crucial role in promoting the physical and emotional well-being of individuals in our care.
Job Overview:
- To provide high-quality care and support services, focusing on promoting independence, choice, and autonomy.
- To work collaboratively with colleagues, families, and other stakeholders to develop personalized care plans, ensuring seamless care coordination.
- To maintain accurate records and reports, adhering to regulatory requirements and organizational policies.
Key Responsibilities:
- Assist individuals with daily living activities, such as personal care, meal preparation, and medication management.
- Provide emotional support and companionship, engaging in activities that promote mental stimulation and social interaction.
- Monitor and report any changes in resident behavior or condition, ensuring prompt attention from healthcare professionals.
- Participate in person-centered planning meetings, working collaboratively to develop tailored care plans.
Requirements:
- Diploma or certificate in health and social care or related field (desirable).
- Excellent communication and interpersonal skills.
