About the Role

Hunter Gatherer Mental Health is seeking a highly skilled and dedicated Community Psychiatric Nurse (CPN) to join our Older Adults Community Mental Health Team (CMHT) in Greater Manchester.

Key Responsibilities:

  • Provide high-quality patient care to older adults with mental health needs.
  • Work collaboratively with a multidisciplinary team to deliver effective care plans.
  • Conduct assessments, develop care plans, and provide interventions to promote recovery and well-being.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in team meetings, contribute to care planning, and engage in quality improvement initiatives.

Requirements:

  • Current and valid registration with the Nursing and Midwifery Council (NMC).
  • Proven experience in a CPN recovery role, preferably in an older adult setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work independently and as part of a team, with a commitment to delivering high-quality patient care.
  • Previous experience working with older adults, with a strong understanding of their unique needs and challenges.
