About the Role

We are seeking a highly skilled and experienced Discharge Coordinator to join our team at Harrogate and District NHS Foundation Trust. As a key member of our Discharge Team, you will play a vital role in supporting the safe and timely discharge of patients from our hospital.

Key Responsibilities
  • Lead on complex discharge planning, working closely with the Multi-Disciplinary Team (MDT) to promote patient flow and achieve the Home First Agenda.
  • Develop and implement effective discharge plans, taking into account the individual needs and circumstances of each patient.
  • Collaborate with healthcare professionals, including consultants, GPs, and other members of the MDT, to ensure seamless communication and coordination of care.
  • Provide leadership and guidance to junior members of the Discharge Team, promoting a culture of excellence and continuous improvement.
  • Contribute to the development and implementation of policies and procedures related to discharge planning and patient flow.
Requirements
  • NMC Registered or equivalent professional qualification in nursing, social work, therapy, or management in care services.
  • Experience of working in complex discharge, with a strong understanding of the challenges and opportunities associated with this area of practice.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to think critically and solve problems, with a strong focus on patient-centered care and safety.
  • Good IT skills, with experience of using electronic patient records and other healthcare systems.
