About the Role

We are seeking an experienced Paramedic to join our Rapid Response Team as a Rapid Response Paramedic - Community Independence Service. As a key member of our team, you will be responsible for providing high-quality patient care and support to individuals in the community.

Key Responsibilities
  • Provide community-based responsive input to patients with undifferentiated, varied, and complex needs.
  • Ensure holistic assessment and community care pathway planning to directly prevent unnecessary acute hospital admission.
  • Work autonomously while drawing assistance from other senior colleagues in primary and secondary care.
  • Provide interventions and make clinical decisions that support patient choice and wishes.
  • Work within the service operation hours Monday to Sunday on an 8 am - 8 pm or 10 am -10 pm shift pattern across the tri-borough sites.
Requirements
  • Appropriate professional registration (e.g., NMC, Adult Nursing/HCPC Paramedic).
  • Experience of teaching and mentoring students.
  • Experience of working in a MDT.
  • Medication management: experience in subcutaneous, IV, IM lines, with administration of medicines in a diverse range of patients.
  • Minimum of 3 years' experience working at Band 5 level.
  • Experience of managing acute episodes of care, cellulitis, urinary tract infection (UTI), falls, functional, mobility, cognitive impairment.
  • Experience of venepuncture.
  • Experience of working in an acute setting with high acuity patients (e.g., London Ambulance Service, A&E, Intensive Therapy Unit).
  • Experience in delivering short-term episodes of care (i.e., from 3 - 7 days).
  • Experience of prioritizing and managing own workload with minimal supervision.
  • Evidence of own Clinical Supervision/Reflective Practice.
  • Experience of safe patient handling.
Desirable Criteria
  • Contributing to a quality improvement project.
  • Community and rapid response experience.
  • Experience in advance care planning and End of Life care, identifying and managing clinical situations working within Coordinate My Care requirements.
  • Experience of leading audit or project.
  • Experience of using Patient Record System (e.g., Systm1, Rio).
Knowledge and Skills
  • Across a range of patient conditions, including common elderly complaints (e.g., frailty, falls, COPD, Hypertension, heart failure, UTI - acute on chronic illness).
  • Knowledge, understanding, and awareness of memory impairment, cognition, and the effects on health and well-being.
  • Knowledge and ability to use NEWS2/SBAR.
  • Knowledge and ability to identify sensory impairment, the impact it has on activities of daily living and the health and well-being of their patients.
  • Ability to know when to discharge and who to refer on to.
  • Knowledge on how and why to raise safeguarding concerns.
  • Knowledge of incident reporting and reviews.
  • Ability to respond appropriately to a complex scenario, using a combination of intuition, assertiveness, and clinical reasoning to achieve the best patient outcome.
  • Ability to manage complaints in a sensitive and appropriate manner, on the phone and in person, providing excellent customer service.
