About the Role

We are seeking a highly skilled and experienced Senior Practitioner to join our East Mental Health Team at Suffolk County Council. As a key member of our team, you will play a vital role in supporting individuals, carers, and their personal support networks through innovative strengths-based assessments and care planning.

Key Responsibilities

  • Undertake assessments and care planning with individuals, carers, and their personal support networks.
  • Develop and implement person-centred approaches to support individuals in achieving their goals and aspirations.
  • Work collaboratively with colleagues and partners to strengthen social work and provide a strong, recovery-focused model for the people of Suffolk.
