We are seeking dedicated, flexible, and enthusiastic care professionals to join our Home Care Assessment and Reablement Team (HART). As a HART Support Worker, you will provide short, intensive reablement support to people with disabilities and those who are frail and recovering from illness or injury, focusing on helping them restore their independence.
Key Responsibilities- Provide individualized support to clients in their own homes, promoting independence and confidence.
- Assist with daily living tasks such as washing, dressing/undressing, showering, bathing, meal preparation, domestic activities, and engaging in community activities.
- Support clients in identifying suitable assistive technology equipment and demonstrating its safe use.
- Advise on strategies for managing fatigue and pain, and preventing falls.
To be successful in this role, you will require:
- A Level 2/3 Diploma in Health and Social Care (or equivalent qualification) or relevant experience.
- Commitment to delivering high standards of care/support in an anti-discriminatory manner.
- Ability to work effectively independently and as part of a team.
- Basic IT skills sufficient to write and maintain records.
- Effective communication and observation skills.
- Ability to remain calm under pressure and manage crisis situations.
This is a permanent position, offering guaranteed contracted hours between 15 and 30 hours per week. Rota shifts operate between 7am and 10pm, including evenings, weekends, and bank holidays. A DBS enhanced check for regulated activity is required for this post.
